WordPress File Upload Plugin (not Formidable Forms)
This uses the WordPress File Upload Plugin… completely different than the Formidable Forms used on the live page. This plugin has no apparent restrictions on file size or type.

Formidable Forms Simplified
This uses the same Formidable Forms mechanism as the live Standing with You page, just no extra fields. Max upload size set in form is 400MB, no restriction on file type.
